Customize Your Benefits

The HumanaOne Colorado Individual/Family plans let you customize your policy based on how you plan to use it, and the plans include preventive care benefits.  Dental coverage and an accident rider can be added to the HumanaOne policies.  HumanaOne offers three plan designs:  Portrait, Autograph, and Monogram.

Portrait Plans:

  • The Portrait plans include the most comprehensive coverage of Humana’s individual policies.
  • Deductible options of $1000 or $2500
  • $5 million lifetime maximum (optional rider to increase coverage to $8 million available)
  • $35 office visit copay ($50 for specialists)
  • Tier 1 generic prescriptions covered with a $15 copay ($500 Rx deductible applies to Tier 2 -4 meds.  Zero dollar Rx deductible available as an optional rider)

Autograph Share 80 Plus Rx and Copay:

  • Two deductible options
  • $5 million lifetime maximum (optional rider to increase coverage to $8 million available)
  • $35 office visit copay ($50 for specialists) – for first six office visits per year
  • Tier 1 generic prescriptions covered with a $15 copay ($1000 Rx deductible applies to Tier 2 – 4 meds.  $500 Rx deductible available as an optional rider)

Autograph Total Plus Rx HSA:

  • Four deductible choices ranging from $1500 to $5000 (double for family coverage)
  • 100% coverage after deductible is met
  • $5 million lifetime maximum (optional rider to increase coverage to $8 million available)
  • Rx and office visit charges go towards meeting deductible
  • An HSA-qualified health insurance policy allows you to put aside pre-tax money to pay for future medical expenses.  See more details on our HSA page.

Autograph Total HSA:

  • Four deductible choices ranging from $2000 to $5200 (double for family coverage)
  • 100% coverage after deductible is met
  • $2 million lifetime maximum (optional rider to increase coverage to $5 million available)
  • Prescription discount card (not insurance)
  • An HSA-qualified health insurance policy allows you to put aside pre-tax money to pay for future medical expenses.  See more details on our HSA page.

Monogram Plans:

  • The most affordable of Humana’s individual health insurance plans
  • One deductible – $7500 for an individual, or $15,000 for a family
  • 100% coverage after deductible is met
  • $2 million lifetime maximum (optional rider to increase coverage to $5 million available)
  • Tier 1 generic prescriptions covered with a $15 copay ($1000 Rx deductible applies to Tier 2 – 4 meds)

  1. It looked like this website was going to list the Humana doctors
    in the Fort Collins, Colorado, area. This certainly wasn’t true. I want to see a different Humana doctor and wanted this
    list. I’ve spent a fair amount of time trying to find this list.

    Sorry about the confusion. At the top of this page, you could have clicked on “PPO/HMO Providers” and at the bottom of the page you could have clicked on “provider lookup page” and they would have given you the option to search providers for any carrier in Colorado, including Humana. But here is the direct link to the Humana provider finder:

    Make sure you’re in Internet Explorer on a PC or the Humana website won’t work.
