[edit] Deductible
The maternity deductible is separate from the medical deductible. The maternity deductible options are $1,000, $2,500, $5,000, and $10,000. Like with the medical benefits, the negotiated provider discounts are applied even before the deductible is met.

The plan pays 100% of routine maternity expenses once the separate maternity deductible is met as long as the providers are in-network.
[edit] Waiting Period
The insured can add the maternity coverage at any time, but the waiting period still applies. There is a 90 day period that the insured must have the Assurant plan in effect with the maternity rider added on before becoming pregnant. (Basically, a full term deliver should occur on or after the 1 year anniversary of the policy)

For a 25 year old female (maternity coverage is not offered to males) in zip code 80020 with no health conditions, here are the prices for all deductible options:
- $10,000 maternity deductible = $15.21/month
- $5,000 maternity deductible = $66.56/month
- $2,500 maternity deductible = $218.71/month
- $1,000 maternity deductible = $503.98/month
These prices may vary by zip code.

The maternity coverage is not available to those with previous pregnancy complications like c-section. Typically, there aren't any health insurance policies available at all to those with previous infertility treatment.
