How to file a claim

99% of health insurance claims in Colorado are filed by the doctors office or hospital because they are part of a PPO network or work with the HMO. If by some chance, you have an indemnity plan or your doctor just won't file the claim for you, make them work for you anyway:

  • make sure they give you a very detailed, itemized bill
  • double check the bill and ask questions about every billing code
  • make copies of everything
  • call the phone number on your health insurance card to have the insurance company help you with what is needed
